If you would like one of the FieldFlō form names to display a different name on the completed PDF view you can change the name from the Form Sequencer Tool.

Stock Form: A pre-built, ready-to-use form that comes standard with the FieldFlō platform.

In order to edit form names with the Form Sequencer, you must have the Edit Form Sequencer role enabled
Here are the steps to changing the name of any stock form:
On your left main menu, click Admin then click Form Sequencer
(Optional) You can use the Filter Form Sequencer field to narrow down which forms you're trying to find; forms that match the search you entered will turn grey so that you can easily locate them.
Now, on the right of the name, you should be able to click the pencil icon.
Enter the new name and then click the checkmark.
